About your Visit

Your health care is a partnership, and you play a key role in that partnership. It is important that you feel free to ask questions, and that you bring any symptoms you might have to our attention. In addition, we ask you to take an active part in leading a healthy lifestyle, including appropriate self-examinations, exercise, and proper eating habits.

Please prepare for your appointment by bringing:

  • Photo ID* (If address on ID does not match address in our records, other proof of identification must be shown)
  • Health Insurance Card/Information
  • A friend or family member to take notes/help with questions
  • A list of all medications you are taking
  • A list of your questions /concerns
  • Copies of records from doctors related to your visit with us
  • A parent or legal guardian must accompany any minor patient (under age 18) for the first visit. Accompaniment for subsequent visits will be discussed during the initial visit.

To protect our patients' identities and to comply with Federal Law, a photo ID and health insurance card MUST be shown at check-in. Failure to provide the required documents will result in your appointment being canceled.

Scheduling Your Appointment

The nature of our practice is to give our patients the utmost in care and service. Please excuse any delays. We will do our best to see you at your designated appointment time as we recognize your time is valuable. We will give you the same careful attention as soon as possible.

As a courtesy to other patients and staff, please call the office as soon as possible if you are unable to keep your appointment or are going to be late. We may be able to reschedule your appointment for a later time that day; however, due to the schedule, this may not be possible and you may need to reschedule for another day. We reserve the right to charge for missed appointments without 24 hours notice.

Contacting Us After Business Hours

After hours, our answering service will contact the physician on call. Please do not hesitate to contact the physician for an EMERGENCY situation. Non- emergent calls to the physician on call may result in a charge. Physicians on call DO NOT refill medications or give laboratory results. Requests of this nature need to be made during regular business hours. If you are trying to reach our on-call physicians after hours with and emergency situation, please call (513) 221-3800.

Non-urgent medical messages

You may submit an on-line, non-urgent, secure medically related question to our physicians and nursing staff. Please include your full name, date of birth, telephone numbers (including day, evening and mobile phone numbers) and brief description of the problem you are experiencing or the question you have. We encourage you to keep your mobile phone on if you wish to be contacted via that means by a member of our medical team. Non-urgent messages will be returned within 2 business days.

Lab Results & Prescription Refill Requests

You can request lab results by calling our office at 513-221-3800 option 2 or go into your TriHealth MyChart account. For prescription refills, please contact your pharmacy.

Our office will contact you immediately about any abnormal Pap smear or lab report, with instructions about follow-up procedures. If we are unable to reach you by phone, we will contact you promptly by mail or through your TriHealth MyChart account.

Please note that normal Pap smear results will be placed into your MyChart account. Also note that mammogram reports will be sent to you directly by the center performing the mammogram in about two weeks. We strongly encourage our patients to contact us if you do not receive your mammogram or pap smear result within that time.

Insurance Questions

You may submit an insurance related question by calling our office at 513-221-3800 option 3 or through your TriHealth MyChart account. Please include your full name, date of birth, telephone number(s), date of service and brief description of the question you have or the problem you are experiencing. An insurance coordinator will contact you.

Medical Records

We can only accept requests for copies of medical records after we have received a completed Medical Records Request form. You can email the completed form to med_rec@cincyobgyn.com, or you may mail the completed form directly to either office or fax your request to (513) 682-4520, Attention Medical Records. Please note, we are unable to email medical records. Patients may make arrangements to pick them up or elect to receive an encrypted flash drive containing the records.

Breastfeeding

A lactation consultant is available for your breast-feeding questions. Simply call (513) 870-7883 or (513) 870-7721 to speak with a lactation consultant directly. If you think you need to be seen by one of our physicians, please contact us.
